About the Bases of Propaedeutics of Clinical Medicine and Physiology

In the treatise main principles of propaedeutics of modern clinical medicine and human physiology are discussed. Nowadays the official medicine considers that these sciences are based on a biomedical paradigm. But the real basis of the medicine (it is understood as the practical science, or the doctoring, science) are constituted byu2026a) the clinical paradigm (it applies the philosophical preconditions of knowledge on the world, nature of person and of the nature formed by person), b) the system of supplementary paradigms among which the basic place is occupied nowadays by the biomedical paradigm, and alsou2026c) by clinically registered data about a current state of health of each patient. Physician applies the philosophical preconditions of medicine, and the clinical paradigm, and the supplementary paradigms, and data of current clinical inspection of the patient. In each doctoring the physician's thinking is directed by interests of each patient in each clinical case. Being based on a clinical paradigm, the medicine explicitly and implicitly develops system of supplementary paradigms. Each doctor reproduces and uses supplementary paradigms in each clinical case by using own professional abilities and possibilities. Each doctor exposes to individuation each clinical case in interests of health of each patient, and uses possibilities given to him (to her) by a society, a science and professional opportunities and abilities.Doctors explicitly and implicitly (at least during 1000 years) use the model of psychophysical unique human nature, generalized by Avicenna as an ontological and epistemological bases of the doctor's thinking. During the individuation of doctoring of each patient the doctor develops the clinical diagnosis which expresses the current nature of health of each patient. Each clinical discipline explicitly or implicitly develops own clinical paradigm that allows the doctor to supervise the own thinking in interests of health of each patient in each clinical case. Thus the doctor considers mental and physical perception patterns by patient of own health status and explicitly or implicitly exposes to individuation supplementary paradigms, first of all - biomedical one, by supervising the current data of clinical inspection of the patient.During doctoring of each patient the modern doctor regularly and, usually, implicitly, uses ideas of Alcmeon, and Hippocrates, and Galen about psychophysical (psycho-physiological) nature of the clinical paradigm, and these ideas were explicitly integrated by Avicenna. But in an explicit kind the modern doctor constitutes judgments in language of the biomedical paradigm.
